Sign up below for the 1st of two COVID vaccine doses.

At this appointment, you will be given the date and time for your 2nd dose appointment (which will occur within a month). 

Please DO NOT schedule a COVID vaccine appointment if…

• you are in quarantine due to high risk exposure. 

• you have symptoms (for example: cough, fever, SOB).  NOTE: If you have symptoms on the day of your appointment, please do not come in. Call XXX.XXX.XXXX to cancel.

• have had ANY vaccine (flu, shingles, etc) in the last 14 days. 

• you cannot return for your 2nd dose.

*To avoid wasting vaccine, it is very important you keep your appointment. Thank you!
