National Childhood Obesity Awareness Month
Healthy people can work; healthy children perform well in school.  A healthy workforce and strong schools result in a healthier community. Welcome to Sunrise Community Health – a community health center designed to support you in your quest for good health by offering affordable, quality care. Our patient-centered care model focuses on providing a sensible integration of the following primary care services:

  • medical
  • dental
  • behavioral health
  • laboratory
  • radiology
  • pharmacy
  • care management
  • health education

As a federally qualified health center, we provide services regardless of a patient’s ability to pay–by screening for eligibility for publicly funded programs or for private insurance, and by offering a sliding fee scale and payment plans. Sunrise’s nine family medicine, pediatric, prenatal, dental and school-based health clinics are located in Greeley, Evans, and Loveland, Colorado. Sunrise is a recognized leader in integrated health services, information technology, community collaborations, and professional health education. Our strategic plan focuses on service integration and expansion for the uninsured, under-insured and publicly insured.  Our vision is a community-based health care system that ensures all have access to high quality, culturally competent, affordable, and sensibly integrated health care. Sunrise co-founded the North Colorado Health Alliance, a community collaboration dedicated to expanding access, improving quality, eliminating disparities, controlling costs, and enhancing the care experience. We are grateful to all of our allies – past, present, and future – for their shared commitment to the health of our community.  If we all do our best, and we do it together, we can heal and keep healthy ourselves and our neighbors.     Sunrise is a Federally Qualified Health Center (FQHC)  Sunrise is a FTCA Deemed Facility (FTCA)
