Booster - COVID19 Vaccine
Sunrise will provide a COVID-19 booster shot (Pfizer) on a walk-in basis at these clinics during regular business hours.

For more information on these boosters for ages 5 through adult, see table below (Children under age 5 years are not recommended to receive a booster at this time.)

Please bring your original vaccine card with you to your appointment so we can determine the correct type of booster shot to give you. You can also make an appointment by calling 970-346-2587.

Children ages 5–11 years
People ages 12 years and older
Children ages 5–11 years 3rd Dose Pfizer-BioNTech (MONOVALENT) BOOSTER. At least 5 months after 2nd dose

got a Pfizer-BioNTech primary series may only get the Pfizer-BioNTech original (monovalent) booster.

Children ages 5–11 years who got a Moderna primary series are not yet eligible for any type of booster.
People ages 12 years and older may only get the updated (bivalent) booster. They can no longer get an original (monovalent) Pfizer-BioNTech or Moderna booster.

Teens ages 12-17 years may only get the updated (bivalent) Pfizer-BioNTech booster even if they completed the primary series with a different product.
3rd Dose Pfizer-BioNTech UPDATED (BIVALENT) BOOSTER At least 2 months after 2nd dose or last booster, and can only be Pfizer-BioNTech

Adults ages 18 years and older may get a different product for their updated (bivalent) booster than they got for their primary series. They can get either the Pfizer-BioNTech or Moderna* updated (bivalent) booster.
3rd Dose Pfizer-BioNTech or Moderna UPDATED (BIVALENT) BOOSTER. At least 2 months after 2nd primary series dose or last booster, and can be Pfizer-BioNTech or Moderna*
